What is Complex Care Management?
Complex Care Management (CCM) is a program designed to support UCLA Health Medicare Advantage Plan members who are managing multiple or serious health conditions - at no additional cost.
Our team of registered nurses and social workers work closely with your care team to help you stay healthy, safe, and supported at home. We partner with your doctors, specialists, caregivers, and community resources to help you better understand your care and avoid unnecessary hospital visits.
We're committed to helping you manage your health - especially when things feel overwhelming. We'll listen to your needs, support your understanding of your conditions and medications, and work with you to create a care plan that fits your goals and preferences.
Support We Offer
Our team provides personalized support to help you manage your health and stay connected to the care you need:
- Regular outreach from a dedicated nurse
- Comprehensive health assessments and personalized care planning
- Ongoing support managing chronic conditions
- Connections to supportive services, including transportation, home health, and behavioral health
- Close coordination with your medical providers
- Help understanding and using your UCLA Health Medicare Advantage Plan benefits and navigating the healthcare system
- Access community resources, including meal programs, transportation, and in-home support
