Disease Management Program
What Is Disease Management?
Disease Management is a free and voluntary program that helps UCLA Health Medicare Advantage Plan members better manage certain chronic health conditions.
The program focuses on helping you:
- Learn more about your condition and how to manage symptoms day to day
- Better understand your medications and how to take them safely
- Receive ongoing support through regular telephone check-ins
- Recognize signs and symptoms that may require medical attention
- Stay up to date with recommended check-ups, screenings, and preventive care
- Help reduce preventable complications and hospital visits
Our Nurse Care Managers work closely with you and your providers to support your care between doctor’s appointments. The Disease Management Program does not replace the care provided by your doctor and does not provide medical diagnoses or treatment recommendations.
Conditions We Currently Support
The Disease Management Program supports members with the following conditions:
- Chronic Kidney Disease (CKD)
- Congestive Heart Failure (CHF)
Your Nurse Care Manager will work with you to provide support based on your individual needs and stage of care.
How Enrollment Works
A UCLA Health Medicare Advantage Plan Nurse Care Manager may contact members who could benefit from the Disease Management Program based on health information such as medical visits or prescription history. Members may also request participation or be referred to by their provider.
