Routine Dental Benefit
Your plan includes dental coverage through Delta Dental's DHMO network, with access to preventive care such as routine cleanings, exams, X-rays, and more. Choose a network dentist and change your dentist at any time. If you have questions about costs or services your dentist recommends, you can request a treatment estimate from Delta Dental before receiving care. Over-the-Counter Allowance Smart Benefits Card &more
Use your $60 quarterly allowance to purchase eligible OTC items through participating retailers in-store, online, or by phone. Be sure to confirm eligible products and participating retailers before shopping. Any unused allowance expires at the end of the quarter.

Post-Discharge Meals
Following discharge from a hospital or skilled nursing facility, you may be eligible to receive up to 28 meals through RxDiet. Choose from prepared meals delivered to your home or ingredient kits with recipes to prepare your meals. The benefit is available for up to 30 days after discharge.

Routine Transportation Benefit
You’re eligible for up to 12 one-way rides per year to medically-related, plan-approved locations and pharmacies through Uber Health. Rides are limited to 40 miles each way and can be scheduled on demand or in advance. Door-to-door and wheelchair service requires advance scheduling.

Routine Vision Benefits
You are eligible for one routine eye exam per year at no cost through an EyeMed provider, plus a $150 allowance for eyeglasses or contacts.
